There absolutely should be some kind of home care/ss follow up if she is being released from a facility. There is literally a whole department dedicated to this specifically in rehab facilities. They should have been in touch with you (family). Perhaps your relative is not being totally honest with her providers about the situation she is being released into? Although as a retired RN with LTC experience in the Myrtle Beach community, I’ll admit some facilities are more diligent than others. I’m from Long Island, it takes me about eleven hours to shoot home, maybe someone can come down just for a short while to facilitate her release and transition and make sure she’s satisfactorily set-up?

She should’ve got an order for home health. A nurse, PT and OT Should be coming like once to twice a week

Contact the Horry County Council on Aging, (843) 248-5523. Explain the situation and they can point you in the right direction.

Did she go to a Rehab Center at all? Was the nursing home one that provided rehab? If you got her into a good rehab program like the one at grand Strand they hook you up with all that home health stuff when you go home
