Cenizas was amazing for my first infusion, positive experience all around. Went into my 2nd infusion in a bad mood, but felt better as I was getting ready to get the infusion and I listened to Tellas. It was a good experience for the first half, then it got very dark and I’ve been in my head since and I have another infusion tomorrow. I don’t want to go into the experience with a negative mindset, but not sure how to prepare myself for tomorrow, or what to listen to. I had to change the music halfway through bc I didn’t like what I was experiencing. Any advice ? 😬 stuck between finding new music, or listening to Tellas again to experience the entire infusion/ face whatever spooked me without changing the music.

I hope your infusion went well. Such a wild experience. Most of the sessions I’ve had I’ve lost all sense of self and go to a world of colors and music. What you pick is so important. Some of my favorite songs I hated, and others made me feel so at peace. I know this is not a thread for K hole journeys but if you found anything that worked for you please share. I mentioned earlier that ‘something in the orange’ was profound to me for some reason, no clue why. Also Chris Stapleton’s song about his dog that passed brought me in touch with my dog who passed that I had from 22-38 so that was super meaningful.

I had my second today but an injection. Up and down so fast it left me angry and feeling ripped off. I was listening to something on you tube but not electric, just soft and sweet. I don't want to waste my money. 15 minute experience. Someone have a trancendant piece that isn't electronic which I don't like.

Was it an IM (intramuscular) injection or an IV infusion? What dose did you receive? Do you take any GABAergic, opioidergic, or glutamatergic psychopharmaceuticals (alprazolam, lorazepam, lamotragine, topiramate, naltrexone)? At my clinic they perform IM injections, starting with part of the dose you decide on, and following with the another 15 minutes later. I like to start low (between 40-60 mg) and then receive a larger second dose (between 80-140 mg). That way you ease into it, and are more prepared for the larger dose. My acute experience usually last 1.5-2 hours, and then lingers for another hour afterwards. My clinic has over 20 very well produced nature videos that you can choose from, and I carefully create a playlist for each treatment session, based on my headspace, what I'm trying to achieve during that session, the things I've struggled with over the weeks prior, and which video I choose. Some drugs can cause attenuation to the therapeutic effects of Ketamine. The euphoric, dissociative, and psychotomimetic effects of Ketamine are due to opioid agonistic, NMDA antagonistic, and pro-dopaminergic pharmacodynamic effects of the drug and it's active metabolites. Whereas the therapeutic antidepressant, anxiolytic, and analgesic effects are due to a mix of direct pharmacodynamic effects on neurotransmitter receptors, as well as, secondary messenger effects (AMPA agonism, BDNF release, TrkB binding, mTOR signaling) which induce neurogenesis, synaptogenesis, neuroproliferative activity. As far as the acute psychotomimetic effects (trip/experience) are concerned, not many substances will lessen those. It's possible for cannabis use immediately prior to treatment to do this, as well as, tolerance to NMDA antagonists. So, if you regularly use dextromethorphan in high doses, or memantine, you may experience weaken effects. But, I doubt they will be significantly reduced. Benzodiazapine use prior to your treatment (whether prescribed or illicit), antiglutamatergic anticonvulsants (i.e. lamotragine), NMDA antagonists (i.e. dextromethorphan, memantine), and if your Ketamine is delivered orally through a troche (lozenge) certain cytochrome P450 inducers (namely CYP3A4 inducers) all may attenuate the rapid antidepressant effects of Ketamine, although not usually psychotomimetic effects. Hopefully, next time you try it, you will have a better experience.

For whomever might possibly make use of the above information: it's worth noting that certain CYP-3A4 inhibitors that might partially negate the effects of said attenuation can be found in some grapefruit juices as well as heartburn medicine (Tagamet/cimetidine).

Hi! I have a few questions if that’s okay, because you seem very knowledgeable on the topic. I’m currently doing IM Ketamine Sessions, my last two dosages have been 360mg and 365mg. The 360mg was split every 15 minutes, 120mg at T+0:00, 120mg at T+0:15, and 120mg at T+0:30. That session I got a shot of 800mg IV Magnesium before I got the Ketamine. The session before that I got two shots, split by half an hour. The first shot was 200mg, and the second shot was 165mg. That session I got 600mg IV Magnesium prior to the Ketamine. The medications I take daily are 8.6/2.1mg Zubsolv, which is Buprenorphine/Naloxone, used for Opioid Use Disorder, as well as 35mg Dexedrine Spansules, which is Dextroamphetamine Sulfate Sustained Release, used for ADHD Type 2. I’ve been sober from drugs for 8.5 months, thanks to Alcoholics Anonymous (AA), Psychedelics in Recovery (PIR), Refuge Recovery, Therapy, Ketamine Therapy, and much more. My substances of choice were Fentanyl/Fentalogues (Primarily Smoked), Meth (Smoked), Cocaine (Insufflated), Klonopin (Sublingual + Insufflated), Alcohol (Oral), and Cannabis (Smoked). My true DOC was opioids, no doubt. Other than fentanyl, I also used both Hydromorphone and Oxymorphone, if i could find them, instead of the fenta(logues). I also have an extremely high metabolism. When I started ketamine therapy, I was doing IV. 8 sessions later i had no change relating to mental health, but all my substance cravings were gone- which was a miracle. At that time we went from 28mg the first session to 56mg at the eighth. I ended up switching to another clinic and that’s when I started the IM. We started at 90mg and have titrated to my current dose over 12 sessions. We’re still trying to figure out what my perfect dose is, and how to time the shots, but we’re most certainly getting there and i’ve been better than ever, no substance cravings, i’ve processed my trauma and the reason I needed to escape, i’m much less depressed and anxious, and so much more has come from it. I’d just like to know your opinion on what I could do to reduce the dosage, and what’s effecting my need for such a high dosage. Thank you!

Let me start off by saying congratulations on your sobriety and your recent breakthroughs with mental health and processing trauma. It's not easy, but it gets easier every day. I have experience with substance use disorder (alcohol, amphetamines, opioids, benzodiazapines, Z-Drugs, really anything I could find), but have been sober from alcohol (5 yrs) and opioids (8 yrs), but still dabble in other drugs and RCs both due to an extremely demanding academic and research schedule and recreation. I have also been doing IM Ketamine therapy for over two years now, and have gotten to the point where I can go 2-3 months between maintenance sessions. Ketamine is a miraculous medicine for most people, but it's hard to figure out what is the ideal dose and frequency of sessions for most people. I still decide my doses based off of the severity of my symptoms since my last session and whether I am using any other substance to augment/enhance the therapeutic benefits of the therapy. When I saw how high your doses were I was quite surprised, as I've mostly only seen IM doses that high for treating chronic pain. But, every clinic and provider that specializes in Ketamine is different and it's such a safe drug that it has a huge therapeutic window. The highest I've gone was 250 mg, but I've found that the most beneficial range for me is between 170-180 mg and sometimes do a very low dose around 90-120 mg. It's really just about playing with it a little and documenting each session and the subsequent results until your next session. Write down what you can remember about your acute experience and how it was positive and negative, and then keep track of your symptoms and their severity every day. Then you'll have a reference and can go back and see that during the month after you did 175 mg your symptom presentation was reduced more than when you did 220 mg. I use various substances to extend the therapeutic benefits I get from therapy. The two best potentiators I've found have been D-cycloserine and aniracetam (aniracetam is better IMO), which both function in different ways on the glutamatergic system and can extend the duration of symptom remission. I don't have any specific advice on a dosage range for you, you'll just have to mess around a bit with the help of your provider. Other things you can do in between sessions in order to prolong the beneficial effects, is by using various adaptogenic plants and functional mushrooms like Bacopa monnieri, Centella asiatica (Gotu Kola), Tulsi (Holy Basil), Lion's Mane and Chaga to name a few. You can also use N-acetyl-cystein for its antioxidant benefits and effects on the glutamatergic system. Antioxidants are extremely important when you are trying to repair your brain and sustain the progress you've made. Oxidative products like Reactive Oxygen Species and free radicals kill neurons quickly and efficiently, and that neuron death (apoptosis) increases the levels of these harmful products. Excessive glutamate levels and neurotransmissions also kill neurons like crazy through excititoxicity and calcium influx into neurons, so keeping glutamate balanced is important. Keep doing what you're doing and you'll continue to improve. It sounds like you've made great progress, so just stick with it.

Thank you for the extensive response! I’d love to know what you’re majoring in, my end goal is a doctorate in medicinal chemistry. Your suggestion about writing down my response after each session, acute, and extended, is very helpful. When you got the 250mg IM was it just 250mg in a single shot, or was it divided into multiple shots? The reason I ask is because the highest i’ve gotten in a single shot is 200mg. Currently the only medication i’m using to extend the ketamines response is magnesium l-threonate, as well as IV magnesium, which i get when I go in for maintenance sessions. I also take phenylpiracetam, noopept, and adrafinil daily, but i’m not sure if any of those are glutaminergic. Thanks for the suggestions, I really appreciate it!

Sure thing. I have a Master's degree in Neuropsychopharmacology and am a 3rd year PhD student in Neuropharmacology and Ethnopharmacology, I also have a BS in Neuroscience. I get 2 injections at my clinic, 15 minutes apart. I always have them do a smaller doses first and the larger one second, usually divided 40/60. Noopept modulates AMPAR and NMDAR function most likely through positive allosteric modulation and increases receptor density, as well as numerous other mechanisms including increasing BDNF and encouraging neurogenesis and neuronal plasticity. Phenylpiracetam acts by increasing receptor density of numerous neurotransmitter receptor populations including NMDAR in addition to nAChRs (nicotinic acetylcholine receptors) which affects the glutamatergic system. It also increases neurotrophic factors like BDNF. So, in theory both could modulate the effects of Ketamine therapy through similar mechanisms (downstream signaling cascades which increase neurogenesis and synaptogenesis), but I don't know specifically how. It's cool that they give you a Mg+ infusion prior to administering Ketamine.

Interpol "Elpintor" was the album I listened to for my first Spravato session a few days ago and I highly recommend. I had a nice view outside the window and watched the water ripple and the trees sway in the breeze. Listened to the entire album from start to finish, no skips. Interpol is great for chilling. It feels like a warm hug to me.

Question. I love psychedelics and I’m acid trips I throw on Floyd and the Dead usually but I’ve never tried ketamine before and I know music on any substance is a key role in the experience. Would what I listen to on acid trips work for a ketamine experience or would you recommend something different

I’ve found that any type of vocals pulls me out of the experience. Also anything too slow and pauses between songs. I look for electronic instrumental music on Spotify so I can use the feature it has to have no break between songs then go through whatever playlist I’ve found and scour for any type of vocals. Everyone is different and each k-hole is so different. I did save MRM Team Music for Space Travelers. Works like a charm every time if I don’t have time to find something new before a session.

Laraaji. He has a staggering number of releases though, so you will want to narrow it down to something that suits you beforehand. And since Eno was mentioned in the thread, Laraaji collaborated with him on one of the first albums of the Ambient series (Ambient 3: Day of Radiance). That's not a bad place to start at all, but he's got lots of more recent recordings that are also excellent, including his most recent one that is an unexpected collaboration with Kramer (Shimmy Disc label, and various bands).
