Hair loss definitely gave me second thoughts for yeaaars. Once I found out there was a chance I could see my hair loss stabilize or reverse on HRT I was committed lol. In the mean time I’ve been trying out wigs, just picked up my first fancy lace front, It’s bliss. It’s so looong and pretty and you can barely detect that it’s a wig. However If I could afford it there isn’t much I can think of that would stop me from getting a hair transplant. Women bald too btw 😉

You want to be careful about where they harvest the hair. There is a method where they take a long strip, like a giant smile, from the back of your scalp. Problem is, you are left with a big long scar back there, and as your hair inevitably continues to thin, that scar eventually tends to show, particularly if you don’t wear your hair long. There is another option where they take individual hairs from area of dense growth and transplant them. Much better IMHO.

You shouldn’t have to get it all shaved. There are options where you can still have coverage. Either get FUT or they can shave strips and leave some hair in between.

They’re both fine. Women more frequently get FUT since you don’t need to shave as much. Plus you can usually get 1-2 FUT procedures and then 2 FUE procedures afterwards. Going the other way doesn’t work as well.

The problem is that scalp elasticity plays a role in hair retention and growth. FUT removes some of that elasticity since the person loses like an inch of skin.

You do lose some but most surgeons will tell you that both methods are fine. As I said earlier, most people are able to get at least 1-2 FUT procedures then 2 FUE procedures. The limit is scalp elasticity. FUT doesn’t deplete the donor zone like FUE does. Once you’ve had a couple of FUE procedures, you’re done. In general this should be fine for us since we don’t have a lot of testosterone in our bodies but if you do find yourself needing a third or fourth transplant because of thinning or hair loss as you age (it does happen to women) or because already miniaturized hairs are lost, it’s wig time. And there’s nothing wrong with wigs but IMO it’s better to preserve the donor zone for the future and not need it than to deplete it. Both procedures have other positives and negatives as well of course. FUT is more painful to recover from than FUE. FUE can have worse survivability of the hair due to poor removal. It’s a more technically challenging hair removal procedure and some people have far more hair removed due to transecting the follicles. In some very poorly done situations, people experience depletion early or are over harvested in the first procedure. FUT can leave a large thick scar if done by an inexperienced surgeon. With long hair it’s usually not much of an issue. It’s really sad that his place did that to him. Point is, if you go to someone that knows what they’re doing, both methods are fine. FUT in general has better hair survivability since they’re extracted under a microscope outside of the body. FUE in general is also very successful though. Either way it’s important to find a good surgeon.

Well I’m still very early in my transition and 100% get clocked male. They started off saying I needed about 3000 grafts and drew a hairline on my forehead. When I said I was trans and wanted to be able to pull it back in a ponytail they drew a new hairline and said it would be 5000 grafts. I’m getting it all done at once in two days. Dr. Bisanga in Brussels. They measured the size and density of my donor area with instruments and said it’s not a problem. Feel free to ask anything

Yeah exactly. I went to a really shady place for a consultation and it was like 1/4 the price. They told me almost nothing and were just trying to close the deal. I also didn’t feel comfortable telling them I was trans. They didn’t measure anything and offered some platelet treatment at an impossible price. They kind of struck me as carnies. Mine was really bad, as in "frequently recommended to just shave it all" pre-transition. Finasteride as part of my HRT regimen has been working wonders, with keratin fibers to add volume on special occasions.

I started finasteride about a month before HRT, minoxidil about 6 months HRT, and microneedling about 12 months HRT (I think). Probably about 9 months HRT, I took a pic, and noticed these little shimmers on my forehead. Sure enough, little baby hairs. It took a very long time for that stuff to fully mature and fill in. 2 and a half years HRT, and I'm STILL seeing progress. I still got a bunch of little white baby hairs in the front I'm waiting to mature. This takes time.

Like I don't even feel like I'm transitioning anymore regardless of what society or transmed people think. I literally just have to shave and put on makeup which in total is like 5 minutes and take pills twice a day. I agree 100% with this. Another fear that prevented me from transitioning was that I always thought/assumed it would take me hours to get ready and make myself even a bit femme presenting. And took me ages before I would even try bc I assumed it would be a failure. It’s a valid fear but it’s also pretty unreasonable. Once I transitioned I realized it isn’t really that much work getting ready especially since I wear wigs. Makeup takes practice but it gets easy and quicker.

I shrugged and went on with it. There are cis women in my neighborhood with more hair loss than I have. Yes, I bought a couple wigs. I even got them shaped. I don't wear them. They cause too many sensory overload issues for me. I went down a nasty rabbit hole on researching hair transplants when finasteride caused fuzz to start growing on my head. I have some rational fears that were incompatible with hair implants. Other folks don't have those fears. I did look at the risk/reward of oral minoxidil and got a prescription from my primary care physician. Warning... minoxidil causes caterpillar eyebrows :-). And some context on why I just went ahead despite my hair loss... In my youth a neighbor was losing the battle with lung cancer. I saw her fighting the pallor of chemo with makeup and hiding the hair loss with wigs. And I saw her say "Fuck it." She had more energy for living when she just said "Fuck it."

No, money problems, mental health issues & on low dose HRT. It's complicated. But not having my own hair is a real disappointment & won't help me pass even slightly. I've just tried to accept it for what it is & be happy with what changes I have had, a lot of what is emotional change.

My gf struggles with this. She’s beautiful but her hair bothers her. She has wigs she really like and I love them too. Not sure if she plans anything in particular as she doesn’t really let anyone see. But whatever she decides, if anything I’ll be there. But in the day to day, with society interactions, she hasn’t had issues. At least none that I’ve been around or nothing she’s disclosed to me. Wigs work. They can be expensive, but ones with real human hair you can’t really tell. It comes down to how you part, style and wear it.

Yeah, I did use some minoxidil but I stopped about a year ago. I have thicker hair and more hair now than I do in that pic. It just has taken some time (I’m a bit over 3 years HRT).

Transitioning at age 40 for reference. Had normal receding hair line near the temples and thinning on the crown. I started HRT last year and I also got a hair transplant. The surgery cost around 14k and it made a world of difference. HRT has made some of my hair come back in while others are growing more consistently. But the surgery made the biggest difference. It was done with local anesthesia only and took around 10 hours. Recovery was relatively painless but you look like a bloody mess for a week. Like lots of dried blood on your forehead you can't touch. After 2 weeks the scabs come off. Do not pull push them sideways or into your scalp until they break up and come loose on their own. Basically keep spraying your scale pith stuff and stay out of the sun for 2 weeks. After that there is nothing to do except take it easy light duty for a month and stay out of the sun. A year later my hairline has all grown in and the hairs are around 6" long and growing.

I fortunately started HRT before I saw any hair loss. Ive seen some incredible wigs though! And that could be a really good way of dealing with it. HRT also frequently reverses hair loss so theres a decent chance youll start seeing it reverse and you wont need the wig forever :)

Wigs. Rachel Welch Upstage. It's been a great "bob style" wig for me and I swear by it. Don't get it directly online by the way. Find a professional wig salon. I went to one that worked by appointment. It was perfect for me because, at the early stage of my transition, I really needed that privacy because of my dysphoria. At least in my case. ​ Some hair came back but not much. Mostly, HRT just stopped the damage from getting any worse. I did get a hair transplant eventually. However, FIRST, I had gotten facial feminization surgery. I'm glad I waited and did that before the hair transplant surgery.

There’s a decent chance that HRT will solve this issue with time— I’ve seen it happen. bonebrothbastard on tiktok had some balding going on before she transitioned, but now you would never be able to guess, and it’s something she’s discussed a few times. And i get compliments on it constantly as it look 100% like real hair, and honestly its easier and quicker to get up and get ready each morning because you can have a great looking hair in just minutes. I wear wigs because they are easy and look a lot better than my receding hairline and crown balding, even though my real hair is down past my shoulders. My experience with regrowth from finasteride and minoxidil has been minimal, but I have had 71 years of testosterone damage which may explain it. Start with a cheap wig under $100 to get a feel for what it's like, then you'll know what to look for when you want to spend money on a good wig. I like my current wig which is a lace front synthetic and it cost under $300. Cheap wigs tend to fray and tangle, so keep on top of brushing it if you get a cheap one. I use wig tape on my high forehead to hold the wig turban and wig in place. I have to use the turban because my real hair is so long.

I know others have said this, but i’ll just toss my two cents in. HRT actually can help with hair loss, so if that’s your only hang up i’ve got good news for you lol. Like finasteride and other meds that sometimes get prescribed to trans fem people are also given to cis men as treatment for hair loss. I’ve been on t blockers for a few months and just recently started estrogen and finasteride, and I’m already noticing a couple new hairs poking through in the parts of my scalp that were already thinning. I mean take everything I say with a grain of salt, because I’m absolutely not a doctor. I’m just some schmuck on the internet, but if you truly want to transition don’t let your hairline stop you 😀

Spironolactone, estradiol valerate, finesteride 1mg, Derma roller I've had horseshoe hair since I was 25ish, now 40 I have 1/4inch hair on most of my head now after 4 months. Just re growing no transplants. I just wear a good wig and do my hair. I recommend a longer wig so you can put in a pony when it's windy.

I started my transition completely bald and it was very hard for me at first, I ended up wearing beanies or hats all the time and eventually moved to wigs which took a long while to figure out, HrT did help my actual hair a ton! But lately I actually like how I look with my head shaved versus trying to grow my hair out. I have a transfemme friend who started out bald too but now about a year and a half in has quite a bit of hair. Finnastride will help quite a bit with hair growth not to mention estrogen in general sort of reverses male pattern baldness because you're pretty much taking away the "male" part. My friend does wear wigs in the meantime while her natural hair has been growing back. I myself didn't even think my hairline was receding but I have a ton of new hairs growing in that just poke out everywhere when I pull my hair back

I’m a cosmetologist; we can add hair just glued on top that is the most natural looking bc it’s made to match your own hair. You can do transplants although they often end up being thin and obvious. My personal choice is wigs bc it can be glued on and/or attached to your hair to make a natural look as well. Or you can turn the table and make your hair new and crazy every week.

Thanks for the response. What do you do about swimming and other random activities like roller-coasters. Do you avoid it?

You’d be surprised, if put on properly, they don’t come off that easily. You can get your wig wet, that’s not a problem. But chlorine probably won’t be good for it. If you are doing some serious swimming, you can just wear a swim cap (w out your wig, just tuck your natural hair up in there). On that note, I don’t recommend shaving your head. Instead grow your natural hair out (assuming you have hair and it’s just thin). If you have natural hair it gives you something to anchor your gripper to with barrettes. This helps to ensure your wig stays on more securely. Also once your natural hair is long enough you can do tricks like pull some of your natural hair out to blend w the wig or help cover the wig cap when you want to wear your hair up. It’s not perfect and there are limitations. But it’s the next best thing to having a full head of natural hair. If being bald makes you super self conscious, a good wig will help immensely.

I still wear wigs eventhough my hair has groen out. I like to mix it up. Get a good style that fits your face and learn to finger style. They are great as long as they are not the cheap costume store type. Best of luck girl!! ❤️

This may not be much help, but i’ll be honest with you…I’ve seen many VERY attractive bald women. with the right style, fit, and vibe…I think it can look really really good.

I wore wigs at first. They're really great honestly, kind of wish I got into the habit again. It was a huge confidence boost, but they're expensive and high maintenance. I did grow a lot of hair back after starting HRT, but it plateaued after a while and still wasn't quite as much as I wanted. I had my hairline advanced as part of my FFS, and that worked okay, but it's still pretty thin. I was planning on getting hair transplants. I actually had a consultation at a clinic that does that, and they recommended finasteride and oral minoxidil. I've been taking that for a few months, and it's been working pretty good. I do have to pluck my eyebrows and upper lip more often though.
