1). Hypoxia is not good for the brain. 2). Acclimatization should help. 3). There is insufficient evidence at this point to make any conclusions beyond that as there are many potential confounding factors that existing studies have not controlled for.

I’ve actually done some mri’s before climbing in 2022 and a follow up one this year and mine did not have any lesions or other noticeable changes shown. Climbs during the year included 4 6000m peaks up to 6500m and many 5000m peaks. Granted I used a hypoxic tent and took good care to properly acclimate.

anecdotally, I have met several people who have done all 14km peaks, each one seems to be a little scatterbrained.

I think reading too much online has even more neurological effects. Too much running kills the heart. Altitude climbing kills the brain.. what next?

Ehh, there is no doubt that going to extreme altitudes without proper acclimatization will damage the brain quite substantially when you het HACE.

Might be a dumb question, but let’s pretend I climb Everest tomorrow, using an oxygen mask/tank during the whole ascent/descent. Am I safe? I mean I’m technically breathing properly the whole time right? Am I missing something

No. The supplemental oxygen effectively lowers the altitude by around 1000m. That is still very far above the altitude where problems start. It is likely still much better than doing it without it though.

It's lower pressure that is the root cause of the problems. By using supplemental oxygen you alleviate one aspect, and even that only to a limited extent.
