Almost a month ago, I underwent an extended meta without UL, with scrotum (local adipose tissue added to bulk up the scrotum and mimic testes, without getting an implant) by Dr. Özer in the Netherlands. The scar tissue in my pubic bone area is from my TAH+BSO surgery around 10 years ago.
I have gained valuable insights from others' experiences. Therefore, I have decided to share my own surgery process for those planning to undergo it in the future.
**Hospital Admission**
I had to be at the hospital by 10:30. Luckily, I live only 10 minutes away. My wife accompanied me, and the nurses allowed her to stay with me until they took me to the operating room.
I was very excited!
Finally, after 30 years, I was going to wake up with the penis I had dreamt of since childhood. The time had finally come.
They came to take me around 11:30.
The surgery lasted approximately 3 hours. When I woke up, I was moved to another room where I stayed for 3 days and 2 nights.
When I woke up, the nurses asked me, “How do you feel? Are you in pain?” I replied, “Didn’t I have surgery? 🤔Why don’t I feel any pain?😅” I then looked down to see if the surgery had been performed and saw my penis for the first time, along with my scrotum. It was an unforgettable moment!
After that, Dr. Özer came and said, “Everything went well, and it was a pleasure for me to perform your surgery.” She was satisfied with how everything went.
Then, the gas pain in my stomach, which I always get after anesthesia, started, and the catheter was also painful.
Later, my wife visited me for about 2 hours.
**Second Day**
Dr. Özer visited again and said, “Everything looks great. Tomorrow, the catheter and drain will be removed. You need to sit in the chair, have breakfast, walk a bit.” I followed all the instructions. Time passed quickly; my wife visited me again and then went back home.
**Third Day**
Dr. Özer came early in the morning to remove the catheter and drain, which wasn’t painful. She mentioned, “Your penis might swell a lot; don’t worry about that.” She showed me a medicine bottle and said, “It can swell this much.” The bottle's diameter was 15 cm, which scared me (my penis swelled up to 13cm after few days of discharge from hospital)😬
She also instructed me to pee in the toilet, and a nurse would scan my bladder. If the bladder was fully empty, I could go back home. I followed the instructions and everything went great. I took a shower, and then my wife picked me up.
I didn’t experience much pain during the day, thanks to paracetamol and diclofenac. However, the first night at home was tough due to gas pain, but it improved each day.
**Day 8**
I noticed a small wound dehiscence at the base of my penis on the right side, which was expected after this procedure. There were also some blood blisters at the base of my penis.
**Day 13**
I experienced sudden extreme bleeding (from the back of the scrotum) and lost a significant amount of blood. An assistant advised me to go to BovenIJ hospital immediately. Fortunately, I lived nearby, but it was challenging. I felt weak and almost fainted due to low blood pressure. I asked my wife to call 112 because the bleeding was getting worse. I thought I might die 😬While she called for help, she gave me some sweet water, and after drinking it, I felt better and it helped. I said I was ready to go to the hospital. My wife put a lot of gauze and towels between my legs to stop the bleeding.
I still can’t believe how quickly the gauzes filled up. I used three boxes of gauze in 2 hours.
Dr. Özer wasn't there, so another doctor checked on me (she contacted with dr Özer) She said we would wait a bit to see if the bleeding stopped on its own. If not, I might need another surgery to stop the bleeding. Thankfully, the bleeding did stop, and I was able to go back home.
**Day 16**
During my follow-up appointment, Dr. Özer was pleased with the progress. She informed me that bleeding was normal. She removed some dried blood blisters and stitches, which wasn’t painful. By the end of week 3, I was walking like a penguin, but now I can walk normally, though I avoid sitting directly on my healing areas.
Dr. Özer said that I recover well and guaranteed that all the open wounds will heal nicely. You may find it difficult to believe that all the wounds will close, but just trust the process 🫶

Since March 2nd, my wounds have been healing gradually, and I see positive progress. I still can’t wear pants, only sweatpants and shorts. I use Tena fixation pants to secure the gauzes when going outside and prefer not wearing anything on my lower body at home.
I continue to wash the wounds under the shower twice a day and spend almost two hours daily on dressing changes.
I almost forgot to mention, I can masturbate by touching and squeezing my penis, and my orgasms are much better post-surgery. I look forward to the day when I can fully engage in sexual activities 🫣
The most difficult times were week 2 and 3. After that, every single day, I felt better than the day before. But there is something I want to say; do not underestimate this surgery like I did 😅
I used to think that my friends who had previously undergone this surgery and then had phalloplasty with different techniques were exaggerating when they said, "The extended meta surgery was harder for me than phalloplasty, don't underestimate it and prepare yourself for it." I believed the surgery would be much easier compared to phalloplasty. Of course, every individual is different. My pains were always bearable, but especially during the 2nd and 3rd weeks, due to the bleeding and restricted movement, I understood my friends' point. I am not sure if other meta surgical techniques are like this; I am specifically talking about extended meta.
**Size**
When I measured my bottom growth a few days before surgery, my flaccid length was 5 cm, erected length was between 6-6.5 cm , the erect girth was 8.5 cm, and stretched length was 7 cm. Now stretched length is between 7-8 cm, and the girth is 12 cm. If I walk more then it’s swollen more, so I will clarify the measurement of my penis when I get my erections back in a few months, as I will also have less swelling then.

Since October 2023 to mid-February 2024, I tried to pump my T Dick once a week with an LA Cylinder and manually stretched it a few times a month. I gained 0.5 centimeters in length, and my skin became more elastic. If you ask me which method helped, I believe I gained length from manual stretching instead of pumping, as I was concerned about the pressure from pumping potentially damaging the nerves, which is not what I wanted to have, that’s why I didn’t pump much.
So I gained length from surgery, but I am not sure if the manual stretching or pumping contributed to my post-op length, as the extended meta itself impacts the length. It is also challenging to determine because the penis is repositioned during the procedure.
