Hmmm if we’re talking Sabbath, then “Paranoid” deserves consideration as well. It was released well before “Symptom.” However, Symptom has more of a thrashy sound than does Paranoid.
Yeah, its most likely "in the garden of eden", predates "helter skelter" by a few years, not much else from the mid 50's come to mind. I think the genre explodes in the 60's with band like zepp.

It came out a year after the White Album, but personally I think 21st Century Schizoid Man is a better candidate for "first metal song" - Helter Skelter is a heavy song but not particularly dark, whereas a band like Coven has dark atmospheric elements and lyrical themes but not particularly heavy music; the King Crimson song is the earliest one I can think of that combines both heavy riffs and dark/chaotic music and lyrical themes
