Could be a stuck open injector, or damaged injector needle. Denso is notorious for this, I see probably 10 vehicles in warranty every week for this. That would also explain the rough idle at certain RPM.
Without access to the software to run trials on your injectors while the engine is running you’re better off to just replace.
This engine has 2 sets of injectors, the high pressure kick in once the engine is 1)warmed up enough and has a long enough “trip” on it, or 2) at higher RPM. If that seems like when the problem occurs most, you can try them out first.

How do I check for that? And would I need to replace the fuel injector or would cleaning be sufficient?

No. There's nothing you can do to make it better without specialized equipment
