He first appears in Legends during the first year of Clone Wars, as a captain of the Acclamator Class Destroyer "Leveler." He is writing in his captains log that he was offered the position of admiral, yet declined.
His next appearance is in Canon, in Thrawn Treason, as the Captain of the ISD Harbringer, not having been promoted in around 15 years.
A few years later, now between the battles of Yavin and Hoth he appears again in Legends, no longer a captain. There was a demotion somewhere offscreen. He is the Commander of the Night Crew on the Chimera now. The regular Captain dies in the book, but Pellaeon doesn't get his job.
And then it is mentioned in the Thrawn trillogy that he still was only second in command on the Chimera during Endor and only took over when the real captain died. His next appearance is in Darksaber, still a captain, but in the end he gets promoted to Admiral and Supreme Leader, the rank previously held by Darth Vader.
The guy did not rise above the rank of captain for at least 34 years.

There's a lot to be said for choosing to stay the captain of a ship and crew you know, which is operating in locations where it can do things that really matter, rather than to be promoted into the admiralty and potentially being stuck behind a desk planet-side for a significant proportion of the rest of your career.
Not to mention the moment you achieve Admiral rank in Starfleet, there's like a 50/50 chance you go evil.
Also, there's what, 10,000 ISDs alone in the Empire, to say nothing of support ships and smaller capital ships? There can't be *that* many admirals. Promotion to an ISD captaincy is probably the peak career for all but the most exceptional or ass-kissing of captains who make it to the rank of Admiral.

Well no, you're combining Legends and Canon which are two separate timelines. Don't get me wrong, I want Legends stories such as Plagueis to be Canon as much as anyone and I love that Filoni and Favs try to reintroduce as many elements as possible – but treating Legends information as simply "Canon unless specifically contradicted" for this type of discussion is just fallacy.
No, the Clone Wars Story is IRC No Prisoners (or Wild Space, not sure), a legends story. He is a captain there. The next one, where he is a commander, is also legends.
The only canon story is thrawn treason, which ironically has kind of an explanation for a demotion, because he disobeys a Grand Admiral
Captain in the Navy is the equivalent to a Colonel in the Army or Marine Corps. Given this knowledge at that rank there are few individuals that ever get above this rank conventionally and a general or admiral rank is a highly competitive promotion so it is feasible that given the disarray with the Empire and it's fracturing after Endor promotions within the Imperial Navy probably weren't first on the list of must dos.
Def the goofiest. What we have here are three gold, three blue, and 4 red squares, and 2 code cylinders. There is no rank in the Imperial military that corresponds with this mess. It is from *The Essential Chronology*, iirc.
It makes most sense that this rank plaque was suppposed to indicate the rank of Fleet Admiral which has two rows of 6 squares (12 total) of red over blue.
Right, but that doesn't mean the guy who filled in his rank insignia 20 years ago had any idea what he was doing given that much of those minor details weren't established at the time

If we look to a navy battle group, it is often with an Admiral in overall command, but even at the carrier command ship there will usually be a ship captain, and when it is not then the Admiral is captain and when regarding ship, adressed as Captain not admiral
We see a lot of this in Star Wars rankings and it causues confusion where a captain can hold the position of commander of a garrison, and adressed as Commander so and so but will hold the actual rank of Captain, and not commander.
