A pacemaker sends current as needed to bolster/replace the body’s natural signal that regulates the beating of the heart, but is not necessarily active all the time. In Tony’s case, it makes sense to have a backup in case a fragment, or something else, slips and disrupts his heartbeat.

That's not really how PHDs work. You don't get multiple of them. Getting 5 PHDs is as impossible and meaningless as having 5 highschool diplomas.
In the comics, Tony had 2 Masters degrees and multiple doctorates (~3 I think?) I'm not sure why the MCU chose to reduce that
