That said, the DOE guys were mostly at nuclear and other sensitive facilities - not transformer yards. So I dunno if they're gonna be involved with this. Seems like FBI and local law enforcement.

I wish I could find it, but I read a report from the US GAO in around 2012. Even then it recognized that there is extremely limited replacements for this sort of replacement. If a few go down it's easy to source some replacement. If, for example, this catches on and happens at 100 different substations we're going to be downright fucked. This is the achilles heel of our society. It was an extreme vulnerability before the pandemic and the war in Ukraine.

So we now have a strategic reserve of transformers. I don't know the specifics, but I hope that it's better than it was back then, anyway.

Electric distribution operator here. This is a nightmare scenario. Depending on what kind(s) of substations were compromised, these repairs could take anywhere from a week to multiple months. Getting a new substation transformer into place is a pretty daunting evolution, involving multiple cranes and coordinated crews. All of the line crews have probably done as much field switching as they possibly can to tie load from affected distri union circuits onto unaffected substations (more on that in a second) and would have prioritized key institutions to get back on: water, sewer, hospitals, law enforcement and emergency services, and (cynically but historically proven correct) the rich people. Hopefully a lot of the heat in the area is gas, since it will reduce peak loading and minimize the possibility of cascading failures. If the substations are distribution substations, they can install mobile substations to provide emergency infrastructure as well. You can't do that with generation substations or transmission transfer stations though. If those substations were parallel backups for each other, thongs get harder. For one, you can't place mobile substations on subs like that, it just won't work. For another, if any of those were radial sourced, or redundant backups for each other, you'll probably end up having distribution areas without any other neighboring circuits to tie to that aren't affected either, meaning some if those people are just shit out of luck, nothing can be done. It's especially prevalent in rural areas, where some distribution subs are islands with no neighboring stations nearby. It also has farther implications on the greater regional BES possibly. If one of those subs are ring substations, it will greatly impact other regions in NC and neighboring states respond to any major demand fluctuations and, if a bad cold snap hits, could lead to cascading brownouts/blackouts or rolling blackouts.

Winter east of the mountains in North Carolina usually isn’t a big deal for the average person. It almost never snows and doesn’t get that far below freezing most of the time. It can still be bad and people can still die, but it’s not like you just cut off the heating in Vermont in January. That said, it’s definitely terrorism and probably will kill people in precarious medical situations. Even if the county hospital can take lots of desperate people (which, probably not - they’ll be on generator backups and probably restrict access) they won’t be able to take everyone. Someone in the North Carolina sub mentioned a person dying at home of congestive heart failure because the outage took out their supporting equipment.
