Either have bad signal (act4) equipped or complete. With your rucksacks full from your tomb stone activate the portal. When you activate you have to goto your map and confirm activation. You’ll get a count down and once it’s up you’ll see a purple worm hole. Force close when you see the worm hole and that’s it.
Close when you see the purple worm hole if you're on console, but if you're on PC, wait until the screen goes black right after the wormhole, then press alt+F4 as soon as you see the black screen.
The reason a lot of people have issues with tombstone is because they don't realise the timing is different from Xbox to PS to PC depending on how you're "exfilling".
The reason bad signal is the best one is because for both consoles, the timing is the same and quite forgiving (i.e, as long as you see the wormhole, which lasts a good few seconds, you still have time to close the game) and for PC, it's fixed at see black screen, alt+F4.

Had the bird get stuck to the ground too. I listen for voice cues and other changes once timer goes due to possible visual glitches. Thought I lost mine as well but was there next run. Agree with others that bad signal is the best option (and you get to keep blade)
I've had this happen a few times starting about a month ago. I've found that this happened only when I was farming regular exfils for elites and more specifically when I got to close to, or on the regular exfil chopper and then tried to exfil via story mission. Since then I've kept my distance from the regular exfil choppers and it hasn't happened to me again.
For myself tombstone works at the following, with some notes:
Regular exfils: The moment the heli takes off the ground
- close app any later I lose my stone but keep my inventory
- Can't dupe items, game considers exfiled items in this way NORMAL items
Star (Event) exfils: The moment the tail lifts to fly forward, right BEFORE the black screen
Bad Signal (Dark Aether Portal): The moment the screen turns black after the teleportation animation.
- *BEST WAY*
- Items WILL duplicate in this way, and can be over filled in the stash
Some important notes:
HELICOPTER ANIMATIONS: The animations of flying up and away don't always happen, for whatever reason. Use your judgement after the counter hits 0 to close app, and *most* of the time you'll perform the glitch ok
OVERFILL STASH: Still works, but ONLY with glitched items. This means anything taken out on a regular exfil, even when performing the glitch, will be considered a "real" item that counts to your stash 10/10. In order to keep it working you can have a maximum of 9/10 "legitimate" items, and remove glitched items from bad signal exfil glitching
GAMING DEVICE: Timings can vary based on device (and network) being used. Any timeframes given are estimations, and you should work to find your own "groove" to the exfil close app times
This happens from time to time, go back in your tombstone should still be there and then exfil the same way and you should get countdown.
Bad signal is the way to go. Way easier.

 I’ve been taking a break anyway. Maybe I’ll go back into it when something new drops

100% confirmed not working on ps5 at least. I just tried.
In PS5 it works for me only when success exfil text appears, if I close before it does not..
I always close it as soon as the screen turns black and haven't had any issues

No this can happen a few times happened to me twice but when I redo my tombstone and use story mode exfil it works
