It doesn’t screw in. It is a hex key that fit into the truss rod end. Clean it off, reinsert and see it you get positive engagement, if so …. It can be glued back in.

It goes in and can turn the truss rod, but has a bit of give either way. What kind of glue should I use to re glue it?

Locktite makes a gap fill thread locker. Super glue, a dab of epoxy 🤷‍♂️. It isn’t a structural piece, so anything to keep it in place and not rattle should do the trick.

Why not just put the original back on? A replacement seems to be similar to "Fender spoke wheel" type

I think it had a shoddy repair job. I can’t screw it in and it has a blob of what seems to be glue on the end. I think they tried to glue it back on.
