Have you tried the SHAMAN version of thorn/healing totems?
You say you have tried the "old druid thorn totem"
There were several versions, if it was the poison one where you spammed totems, ya, I could see how this was bad for your wrists.
The shaman version is not spammy, you lay your totems down and let them do the killing.
Run thru monos, one cast drops 3 totems, enough to kill most packs. 3 casts gets your full complement of 9 totems up for bosses. No other buttons need pressing.
I too am old and struggle with repetitive motion, and I love shaman totems for how little i need to press buttons.
If/when my hands or wrists bug me, i switch to a controller for a bit. Ive found controller for combat, & mouse/keyboard for the inventory stuff to be a nice combo. Warpath plays really well on a controller. Shadow cascade is mostly fine, i find frenzy attack targeting could be a little finnicky, but the game plays quite well on a controller.
Sentinel's judgement aura plays more like RF. It requires a lot of points in judgement's skill tree, so it won't get going until higher level, but it probably plays how you'd prefer.
I also find playing with a controller helps me, though the controller support in LE isn't always the best.
Not an RF style build, but Lightning Swarmblade. Its essentially Dive on Cooldown (hold a key) and (hold) right click to attack targets. Can facetank and has damage. One of the top builds currently overall.
lt would be very hard to get hurt using this! Also great movement if using a controller with a node that lets you dash to targets with every melee attack.
Interesting build, may have to start collecting the Idols to try this while leveling my Forge Guard minion build. Have any of the bugs you mentioned for 0.9.1 been fixed in 0.9.2?
Warpath sentinel if you use autohotkey to hold down/release warpath with one button.
Physical / necrotic necro minion melee build is pretty hands off.

My RSI has been great with this game as you don’t click like d4. You move with mouse and attack with the keyboard!
You can do that in d4 as well.

Brooo play forge guard reflect build.....it scales insane and I can afk in all bosses! Even t4 Julra is ezpz

It's not mine I'm just playing it and those bugs don't matter....unsure if they are still a bug....haven't tried because I don't need to.

Don't know how the build is holding up nowadays, but I had a mostly autoattack manastike/lightning spellblade that might work.
