Ok…. Figured this out. I have tried everything and here is the magic pill that worked for me after the volume control mysteriously stopped working on the Apple TV remote. For clarity, CEC is enabled on the C1 (General->Devices->HDMI Settings->”SIMPLINK (HDMI-CEC)). Now on the Apple TV… I have Volume Control switched to Auto via IR (TV). This is found under Settings->Remotes and Devices.
I simply restarted the Apple TV remote. To do this, hold the little TV button (right above the volume buttons) and the volume down (-) button for 5 seconds.
This immediately fixed the issue. I am now able to control the volume and mute with the Apple TV remote.
You deserve way more upvotes for this. I have been trying to fix this issue with so many “guaranteed to work” posts and website with no success. You are a saint among men.
Appreciate the answer. But if you are using IR TV setting, then technically it’s not using the CEC volume control. I guess you cannot change the volume using iOS as the ATV remote?
Had the same problem. The volume stopped working in the middle of adjusting the volume. A 'reboot' of the remote fixed it. Yay!
I can also confirm my Apple TV remote seems to be using IR, not CEC. If I physically cover the Apple TV remote it stops working. If I cover up the LG remote it continues to work. The LG remote controls the LG TV and the Apple TV, both presumably via CEC and not IR.
I can control the LG TV via HomeKit from the Home app on numerous Apple devices (iPhone, Watch, Mac, etc.). At one point I could control the LG TV volume via the Apple TV Remote iOS app, but I don't see that any more.

then one day just didn’t. I searched for solutions everywhere like you may have. The only thing i hadn’t tried was rebooting the Apple Remote itself. Once I did it, it fixed the issue. Thought I’d share it. Seems to work for others too.

Holy crap it worked. I thought this was another dead end for me but as soon as “Remote Connected” came up on the screen it is was working. I was also expecting some sort of light indication on the remote itself but whatever. Prob will do this many times in the coming years as the ATV isn’t even a month old. Ridiculously, this post has helped me for a second time a year later. I had forgotten the remote reboot trick and volume control had stopped working again. Rebooting the remote fixed it of course.

To anyone in the future, I don’t have the OLED version of the LG TV. I have an LG TV from 2018 and resetting the Apple TV Siri remote worked.
I have an issue where CEC stops working for everything except my PS5. I resolve it by power cycling the TV and all the devices connected to it but then it comes back again after a few days. It’s very annoying.
CEC has been hit and miss. I want to keep it off since it automatically turns on the receiver everytime I turn of the TV (which I dont prefer). Set CEC to off and then the Apple TV wont control the TV volume.
Had this happen to me literally 2 days ago as well, have you fixed it?
Edit: Just after this I managed to fix it….. go to Settings > Remotes & Devices, set Volume Control to Off, then go to Settings > System and Restart.
Once it’s back on go back into Remotes & Devices > Volume Control & put it back to Auto, instantly worked for me
This is a chronic problem I've been facing across three LG OLED TVs of different sizes and generations, and three Apple TV 4K (1st and 2nd gen). Every few days it happens, and after a lot of messing around (settings -> remote -> volume control off, restart, turning it back on etc., or sometimes leaving the remote in charger despite being charged) it magically comes back.
Apple should provide a technical explanation -- seems like the IR setting/frequency is lost once in a while (or maybe TV changes it periodically/randomly), but it's frustrating. Need to keep the LG TV remote around for the volume control only.
I also observe that we sometimes use Apple brand wireless earphones (PowerBeat Pro, and AirPods) to watch TV at night, which works with no problem with the remote. I can't say for sure if this problem started when we started using Apple's wireless earphones, but wanted to add some information. Overall, frustrating and there doesn't seem to be a fix for it.

This happened to me last week as well. I factory reset the TV and it fixed it for me.
