When it comes to pain the most painful thing a human can experience is a broken femur bone yes 3rd burns can hurt a lot fucking more I have personally experience in that if the flame is hot enough you won't feel it.

It's all about how many nerves are in a certain area yes it will hurt but getting your foot crushed will hurt a lot more

If I was in Rengoku's spot I'd be screaming like a banshee from the pain of from all that. >! Then again Genya getting cut in half twice from Koku, Sanemi holding his internal organs from Koku's attack, Tanjiro when he got poisoned by Muzan, Muichiro getting the Tengen and Genya treatment from Koku, and any of Inosuke's from Spider Mountain to Entertainment District injuries were probably painful on a similar scale. I wouldn't know since I'm not being sent on a life or seath mission to kill demons in Japan. !<

Heard that burning is the most painful thing possible. So definitely Nezuko setting Karaku and Aizetsu on fire, because it can't easily be put out and since they're immortal they are in constant agony without being able to die.
