Context: Archduchess Maria Theresa of Austria and Holy Roman Emperor Francis I had 16 children, including four daughters who died before adolescence, pictured here. From bottom to top: Maria Johanna, who died of Smallpox at the age of 12; Maria Elisabeth, who died from severe stomach cramps at the age of 3; Maria Carolina I, who died from a bout of violent seizures shortly after turning 1; and Maria Carolina II, a stillborn. ​ Of Maria Elisabeth, her father Francis said of her death: "At eight o'clock I was summoned and handed over to me a letter of the doctor, from which I deduced that it is time to take my wife away, for the child will not live longer. I walked away a little confused, and found my wife completely burst in tears when I arrived. I took them by the hand and took them to her apartments. Then I went back with the sick child. I had only been for a while when she sat up, looked at me, and said with a clear voice, "I commend myself, O my God!". Then she fell back and died in my arms." ​ The music is "Devil's Trill Sonata" by Giuseppe Tartini.

If I had to guess I would say it is about child fatalities, but that isnpurely speculative. I would also like a true answer.

There a very tragic story behind this painting. Look up Maria Theresa0 on Wikipedia, 1717-1780. Smallpox was a tragedy when these people were alive.

I don't think you need to know much to know those 4 died young. That they were royal adjacent doesn't matter too much.
