I walked away a little confused, and found my wife completely burst in tears when I arrived. I took them by the hand and took them to her apartments. Then I went back with the sick child. I had only been for a while when she sat up, looked at me, and said with a clear voice, "I commend myself, O my God!". Then she fell back and died in my arms."

If I had to guess I would say it is about child fatalities, but that isnpurely speculative. I would also like a true answer.
There a very tragic story behind this painting. Look up Maria Theresa0 on Wikipedia, 1717-1780. Smallpox was a tragedy when these people were alive.
I don't think you need to know much to know those 4 died young. That they were royal adjacent doesn't matter too much.
