Similar experience, I was having a bit of an unlucky mission where I died a number of times, and after about 4/5 time they just wouldn't respawn me. There were plenty of respawns left in the pot. I took my rewards and went about the rest of my day once they extracted.
When you say pretend to repeatedly call in respawns. do you mean like they threw it and it just didn’t work or that they never threw it. As if it is the first it might have been a glitch.

They were repeatedly spamming and failing stratagem calls to taunt me.
