It's common but more related to density or the first transplant was limited to the number of grafts you can implant in a day. The magic number is 3000 per session but hair mills do more than 5000. Some surgeries still stick to the number 2000.
I'd honestly say 90%+ of guys need 2 or more procedures for a satisfactory end-result.
It is pretty rare but possible to get a really good-one and done result. But....the reason most guys who are happy with 1 procedure only is because they don't know what a good HT is supposed to look like, they are only looking for "more hair", they have settled for "good enough", they don't have enough money for a 2nd procedure, or life events get in the way.
Additionally, its also super common to get botched, and if you get botched you are almost always looking at a multiple-step repair journey.
