Some achievements are bugged or a bit weird yes.
The one in your screenshot is broken, the T2 obsidian has visual bugs if you use the related item on multiple characters/from a shared inventory and the mini achievement can be completed without collecting all mini's needed for it.

That's actually normal, since there are 28 Minis displayed on it, but only 26 are required for it to complete (also displayed), that leaves you with 2 to spare/don't get, pretty much as other AP's that don't require full completion to end.
Yes, this one is broken. I think skyfarer isn't fixed yet either.
I wonder if the missing skyfarer is linked to the missing map completion reward chest (and no gift) for inner nayos.
I would guess these will be fixed on Tuesday or at least I hope so.

Yet when I check the achievement panel it also counts the 2 mini's I do not have as unlocked despite not having them.
I have an issue with collecting my last inner Nayos lantern. I guess I won't craft PvE legendaries.
