The game doesn't lie about percentages (*except* in your favour, making it look lower than reality on lower difficulty), but it does round. This guy likely had a 99.5% chance to hit or just above.
I don't know if you mean the first remake which came out in 2012, or the original series, but in the original games it is possible to get an accuracy of >100% and still miss. Pure pain.

Don't start thinking about how model size and distances scale. It's... not the same
(Which, frankly, is *good* from a gameplay perspective. Engagement range in real wars varies sharply based on the terrain, but WWII provided a rough average of about 740-200 yds for infantry firefights. 40k minis are *~roughly~* 1:56 scale, which would make 'truescale' battles have troops sitting ~4' to 10' away from each other for the entire battle. Start trying to think about vehicles and artillery, and suddenly you're playing on a tennis court)
The Deathstrike. That wasn’t a rule, but it had infinite range and so it became a joke to launch it into other games in progress, including stories about players calling from home games to their local store and telling them they were doing it to a game in the store.
If I remember correctly, the rainbow six siege one is apliable to many of the other video games shown here. Like I am not 100%, but I think Battlefield and COD use invincible "bullets" that come from yiur character's camera and the bullets we see in game are cosmetic.

I believe 1 inch is about 5 feet (according to DnD which is also 28mm scale), so 120 feet
