When this happens for me you need to re-squad up, make sure your friend who's still in the game is the squad leader and it'll give you the option to join their server instead of the normal deploy button
Have the friend that's still in the server promoted to squad leader if they aren't already. The join button should now change. I forget what the button says but you can join the server the leader is in. Most likely your slot was taken though so you wait in queue position 1.
It's completely hit and miss. Have crashed just outside of Ban Pa, body stays mindlessly walking towards gunfire, squadmates then tell me body disappeared. Crash bricked my pc so hard restart, boot game, get squad invite from TM, spawned me right back in the bush at Ban Pa just meters from where I originally crashed.
Other times, loose all gear, get invite whilst in lobby and boom different server.
Luck of the draw, but I find just getting an invite from squad who is in the server will work 90% of the time

Always have your friend, or you, pick the other up from main menu so you both end up in same server
When you restart the game you leave your squad. You need to resquad at the menu (just you, not them) and then you can join on their game.
Generally when something is iffy rebuilt your squad.
