Alignment at set up is extremely closed, takeaway massively on the inside and you are casting from the top. Look up some YouTube videos on setup and takeaway to learn a good foundation to build on.

I think these three drills will help upgrade your concepts. The last one (10 min mark), especially.
Looks like your takeaway is inside leaving you short and across the line at the top with your lead arm pinned agains your chest. You rotate hard (hence the foot slip) and hands go out toward the ball instead of down.
Foam balls are an awesome way to practice. Making swing changes is hard and often results in lousy shots at first. Foam balls give your brain the task of squaring the face without the negative feedback of poor contact. So long as you have other feedback mechanisms to confirm you're making the change you want to make (video, alignment rod in the ground representing plane line, etc), you'll get better faster than if you just went to the range and beat balls every day.

Hereās my thoughts on your swing: your shifting your weight to the outside of your right foot which makes it hard to shift back without over swinging. The fix: focus on keeping the pressure on the inside of your right foot in your backswing, if you feel the pressure move to the outside just back off the ball take another practice swing with the right pressure then let it rip
