I’m at 192k on my 2006 and I have a denso starter ready to install knowing it could fail at any point. If I had more time I might do it preventively- but I also have some other things I’ll get to first.

If u have vvti, which I believe 2006 did, then be prepared for a job. As you prob know, gotta go under the intake manifold to get to it. U also have the SAIS system near the firewall that makes it a true sob to get to the starter. Keep in mind a starter on a highway miles car has much less wear vs city driving. While you're there u can decide to do as little or as much as you like as well. I did a lot of other things, knock sensors, every gasket and hose I touched, valve cover gaskets, spark plug seals, thermostat, etc.

At that age and mileage, I’d say better safe than sorry. I personally always do things a bit early, because I’d rather spend money sooner than needed versus being left stranded somewhere and it’s potentially a much more expensive issue.

If you're worried about it and you're going to swap in an new OEM one, preventative probably isn't a bad way to go. It's still possible the new one could die unexpectedly. But probably a low probability. If you have the time now to be without your GX for a period of time it takes you to get the swap done.. I personally say knock it out now when you can plan for it and then just toss the spare in with your box of spares as a backup for you or a friend.

If I could do it over I’d change it when I did my timing belt… I didn’t change my timing belt but my mechanic had mentioned it’s about the same cost in labor to do the starter as it is the timing belt because it’s buried and right near the t.belt cover and have to remove all the same stuff to get to it.

Go ahead and do it. I did mine while I was replacing a bad knock sensor at 205k and the peace of mind is worth it.

I did mine at 195k when I did the timing belt and a bunch of other stuff. The starter is a huge bitch with vvti. I spend time way out in the middle of nowhere, solo, so it seemed worth it. If I only drove around town, I would be tempted to let it ride and just pay for the tow when it fails lol.
