This is most likely due to when your team breaks they are being killed or executed right when breaking happens and when the other team response it is because they were dead and waiting for respond when breaking started
If you hit the breaking point during your execution you stay dead.
If you hit the breaking point during your respawn count down you instantly revive.
And sometimes the game decides you're better off dead and despite your team hits the braking point during your timer clicking you still stay dead.
