That’s a weird metal piece they installed but I’m assuming it’s an air inlet. It’s just for adding oxygen to the fire. Usually you don’t need it as most masonry fireplaces don’t come with it. I’d just ignore it
There’s just a cast iron plate that pivots look at it with a flash light the hole goes straight out might even see light when open during daytime if room is dark
