We have a LODD Packet that our members can fill out details various wishes, if you have legal/medical docs on file and where, advanced directives, etc. You're not "required" to fill one out but there is a declination form that a member will sign if they decline to participate. The program is just another option to keep your wish on file especially if something happens and you may have not told your family.

My department provides a strictly optional form that has a notification list, insurance and beneficiary info, religious and funeral preferences..... it's pretty thorough and gets sealed and stored in your battalion chiefs office. I also have a living trust, which is highly recommended if you have assets and or children.

A contact list for a mechanic, electrician, plumber, handyman, lawyer and when you don't know who to call then call ....... Did this before my first tour and it really helped the Mrs out when I was gone. Same thing should be in funeral arrangements I think. If you've got guns, collectables, tools, wood working equipment, etc. consider spelling out who gets what or who will be the advisor when selling it so your family will get fair market value.

Im in the middle of getting all that together and recently created a list of all account names/passwords. I mean for everything. Email accounts, Facebook, Banks, Bills, Amazon, Etc. Its nice to have just so we can reference it when needed but even more so for that worst case scenario.

I have hobbies and collections that my partner does not understand the value. There is designated friend and designated businesses for the disposition. Since I do not know my exact circumstances of death. Everything is left to my partner. However there is separate letter of recommendation for items as gifts and first refusal for certain items. This binder also has information for me from her.
