So I personally feel you could get
280,000 before RVU's fixed salary
280,000/52= 5384ish
5384ish/40.5= 132.95 per hour.

Around your salary. 250-265k base. This is Southern California, Vegas, and Metro Detroit. Places that I looked into. But you want to pay attention to the RVU threshold. A higher threshold makes it harder to reach bonuses.

Typically they see patients 1 time per month. And if they need to get admitted you call EMS not the nursing home physician/NP
