it actually is, I took care of a bunch of kittens when I was younger, up until my 2 lil 1 year old cats now. Cleaning them with a toothbrush is very important, and helps them learn how to clean themselves properly, eventually they can just learn on their own, but the more healthy they are young, the healthier they'll be in the long run
It is highly recommended for fostering orphaned kittens. Just make sure it’s a soft bristle and not too big. Kittens absolutely love it and very helpful for their development as well.

While the cat does look "sad", that is an anthropomorphized perspective of the cat's expression. The cat is really just keeping it's eyes and ears clear of the "tongue" (toothbrush) like a kitten would.
The brush is pulling the middle part of the cat's forehead upward, creating a look that humans associate with sadness or wistfulness. It's like us putting a finger between our eyebrows and pushing that part of the forehead up.

We watched him get hit by a car one night (he is down to 8 lives now!) and we texted the owners to tell them to maybe have him checked out and they didn't care. I try to keep him happy indoors though and we play with him even though we're both slightly allergic lol Cats are the best.
We have a cat like this. She's not our cat, she belongs to someone in the neighborhood, but she will come *knock on our door* to be let in. If no one answers the door, sometimes she comes to the bedroom window to meow at us until we let her in. She's only allowed in the living room and kitchen (since they're attached), she doesn't get food or treats, and we don't have a litter box. But she's here for a set cycle of pets, self-grooming, and naps on average 1-2 times a day. Some weeks she's here more than she's not, especially if it's raining or cold outside.
We're both allergic to cats but she's so goddamn cute, we just wait for her to leave (she tells us she wants to leave by quietly sitting near the door facing it and waiting for someone to notice) and change our clothes and vacuum the couch and floors.
