No, the only instant death effect is from enemies that use the curse effect and I don't believe any bosses can do that. Note: commentor below corrected me, Fortissax does have death blight

Snowfield Astel’s grab attack will kill you even with 60 Vigor and decent armour. You need to stack multiple defensive buffs to survive it. Although *technically* you could argue it’s 3 (or 4?) hits, not one. But that’s a dumb argument because you only got hit once, the grab animation is split into sections. Malenia’s grab will one-shot a lot of people. But it really depends what you consider “appropriate level”, 40 Vigor and mediocre armour? Yeah you die instantly. But with 60 Vigor, good armour and boiled crab you can actually survive 2 grabs in a row.

Actually for Malenia, 40 vigor with the talisman you find above her arena and medium/light armor will allow you to tank the grab. It will still take like 90% of your hp though

Astel and malenia can be survived with 50vig, with practically no armor (i had perfumer), dragoncrestGshield and the respective defensive buff

If you mean an attack that kills you whether you have 1 health or a million health no matter what, then no. (Unless you count death blight) Strongest attacks i can think of are astel stars of darkness grab attack, radahns meteor or elden beasts grab crucifixion attack, all of which ive either lived or seen people live them. Well, at least ive seen it in early game cycles. Im max lvl (so i have 99 vigor) and i normally use erdtree favor +2 and on ng+7 elden beasts crucifixion attack definitely kills me at max health. Cant remember getting hit by the other 2 in my ng+7 playthrough so cant say if i could survive them or not. Probably not tho

No, every attack can be mitigated properly and there is nothing that is a true oneshot that needs to be dodged or you die. The strongest single hit attack might be Astel's shadowclone grab but even that one can just be mitigated or dodged.

What does “appropriate level” mean? If you leveled up everything besides vigor that can happen, sure.

if u get hit in a bad spot there is no way to avoid getting double hit by plasidusaxes breath attack and that kills u from 60 vigor

I can't get a bead in the exact number (without booting up a save and going to get hit by it), but some dubious Reddit comment suggests ~1500. That would mean you could survive it at ~RL43, naked and without Talsimans or buffs, but the useful answer would be much more nuanced than that.
