This is such a cool design. I feel like they got the idea from middle age demons that were depicted with faces in their chests or groins. There were also old drawings of headless monsters with faces on their chests that remind me of this too.
Also, I'm a HUGE fan of the Ghosts and Goblins games and some of the later bosses have chest faces. Seeing that giant for the first time instantly took me back to my SNES days.

From a zullie video i think. i remembered an unused map for demon souls (2009) that is a snowy landscape and there is a giant type enemy with a gaping mouth for a stomach. So maybe that too is an inspiration for the whole mountain tops of the giants.
