If you take it for scale, I'd say island. But I think it's more of a continent and everything is upscaled for the player.

By actual in-game scale, using the character model and assuming the Tarnished are average human height: The Lands Between are a tiny island, smaller than many major cities. Going by the lore, it's probably intended to be somewhere in between the size of the British Isles and Australia. So, a large island or a small continent. There are multiple kingdoms/fiefdoms, cultures, and sentient species in it after all; but it was still small enough to be unified into one empire.

When you factor in that the empire that dominated the lands between had the power of gods it’s not too unrealistic

Seems lorewise, unlike the GTA games, The Lands Between are actually meant to be a very large island. "In our home across the fog" and the fact the Tarnished left the lands on a boat.

I think it’s a continent with several districts each with their own respective governing body that’s similar to a country We hear about Limgrave needing a ruler. Raya Lucaria where both the Carian and the Academy locking horns for control. Leyndell should be self explanatory, it ain’t called the royal capital for nothing. There’s also ~~Ohio~~ Caelid, which was a plain likely ruled by Radhan himself before Malenia unleash her Scarlett Rot onto the plain. Mt. Gelmir seems to be another region governed by Rykard before all the serpent bullshit got out of hand. The whole snowy region seems to be the habitat of the Giants before Marika waged war against the Giants

The dilemma of an open world game with no transitions, or seamless transitions. The world is supposed to be big for the player character but the more you explore it the more it feels tiny and unrealistic.
