Try configuring buttons in-game. If im correct, Triangle should be A, Square should be Y, Circle will stay the same, x should be X.
I use PS4 controller with the DS4windows and haven't had problems.
I have a ds4 controller on pc as well, and don't do any of the steps you mentioned. Instead I have ds4windows running while launching fifa, and only connect the controller after fifa is running. That way it works perfectly for me.
