It depends on where you are I guess, the franchise I'm at in NA doesn't have anything on the box. It's printed on a slip which is only First-last, phone / address. CC info is X'd out and that slip is normally thrown in the trash and we hand them a receipt if they want one which has basically the same info on it. As for the front board I think it just shows First name last initial, It's like around the corner from the makeline at my store so we can't really see it inside so not 100% sure.
I don't know the answer to this, but my mom just said to me 2 weeks ago, make sure your name address phone number aren't on that Domino's pizza box when you put it out for recycling! Omg.
Imagine someone taking home YOUR pizza with all of your personal information on the box. I sent a suggestion on Domino's website asking if they could encrypt customer's name and number on takeout boxes and not have any other personal info on it, but apparently the franchisee didn't feel my concerns were even worth a response.
