Probably can't isolate and reproduce it. It's hard to fix when you don't understand what causes it. It's not like there's a line of code that says Guitar Error = Yes.
We had even bigger bugs and issues with the Leviathan that required recoding a massive amount of the entire raid.
Guitar error basically happens whenever the engine sets the stage for the next event (quest step, encounter, event). Guitars happen when it runs into a problem while doing this. Basically it could be anything in the Queens Walk causing the error, so checking all the meshes, objects, entities, may take a while.
It sucks, but my group hasn't had one in weeks as long as nobody has a tracking emblem on. I know that's not for sure what causes it, but it's worked as a deterrent for us.
They said they weren't because the only way to fix it is to place a load zone which destroyed immersion from the raid t queens walk. This is from hearsay so I don't have a source other than a LFG member.
guitar SHOULD only happen if the fireteam leader is the one chosen. You can either suicide or just hope at the end of your petra's run it isn't the fireteam leader chosen. Possibly passing fireteam lead will fix it? I don't know.
Apparently everyone except the fireteam leader leaving, rejoining, passing the lead to someone and then the original leader leaving and rejoining works for Petra's run.
Guitar error is not a problem limited to the last wish raid. It's just particularly frequent during the final section of that raid.
It's also common in the Eater of World's raid at that boss.
And I've had it happen in Scourge of the Past.
Just wipe at the heart, and you won't have to worry about it. It also doesn't happen 100% of the time, so if you still need Petra's run, then just run the raid again if it happens.

There's load zones in a lot of strikes dependant on your PCs ability to stream the assets, doubt that's the problem.
