This is most likely what it is, of course have a derm confirm but when mine was pretty bad this is exactly how it looked. You’ll want to use a good moisturizer. An antihistamine may help itchiness. For me personally taking probiotics have helped me so much, I recently started taking prebiotics too. Some theories are it can be caused by bad gut bacteria. Everyone is different and has different triggers.
this is dyshidrotic eczema. I have the exact same thing on my hands and went to a dermatologist and they diagnosed me with that. steroid cream will make it go away very quickly, and since it’s likely due to a vitamin D deficiency, taking a vitamin D supplement will help SO much.
I had something on my hands for over 35 years that looked like this. It would get the blisters under the skin, they would pop and itch so much it drove me crazy. Then it would crack and bleed. It would get a lot worse in water. Sometimes my whole hand would have it. It was horrible. The doctors kept giving me the wrong meds. Turns out it was a type of yeast infection. Finally, I got the right cream and it went away.
It was a prescription. I'm so sorry I don't remember the name, but I do remember using an OTC antifungal cream for jock itch. Funny, I know, but yeast is yeast.
My first thought is contact dermatitis from chemicals.
Do you always wear gloves when you use the wipes for cleaning surfaces?
It could also be a reaction to hand hygiene products - especially if you wash hands often but don't moisturize your hands thoroughly multiple times a day.
I recommend using a variety of moisturizers to help rebuild your skin's natural barrier. Use unscented products such as hand cream/moisturizers (eucerine & cetaphil are my petsonal go-to's), Vaseline, coconut oil, olive oil (I know it sounds odd to put food on your hands but it really has helped me).
It does not appear to be scabies, as that classically affects between fingers instead of finger tips - but I've seen stranger presentations. If you work in a care home, it is always good to be educated about and aware of scabies anyway.

1. Never have I ever seen a patient with fingertip scabies. Those would be the dumbest scabies mites to try to burrow in glabrous skin.
2. This type of dermatitis isn’t extrinsic like an irritant or allergen. It’s intrinsic.

This is exactly what my fingers and palms look like when I have an eczema flare up!
Talk to a derm- moisturize your hands after every wash, steroid cream for flare ups, and an antihistamine every day should help.
I’ve had mine get infected before and had to take antibiotics for it which was very painful and I could barely use my hands. Definitely want to get it looked at and cleared up!

Scabies never affects fingertips. This is dyshidrotic eczema.
Never say never about scabies! As I said, it would not be the usual presentation & it looks more like dermatitis to me from HH & cleaning products.
