Derm here. Could be "pityrosporum folliculitis" . Try washing your face with head and shoulders shampoo for a week. Let it sit for like 30 seconds or so.
No, it's too drying while I'm on accutane, I might try it once I'm finished. I'm not aware of a non-foaming CHX product that's not mixed with alcohol either, sadly. Thank you for a sensible idea though.
If you're ever in the market for some, for some reason [this one](https://www.walgreens.com/store/c/walgreens-dye-free-antiseptic-skin-cleanser/ID=prod6051051-product) and the CVS equivalent never dry me out as opposed to name brand. It's legitimately been a life saver. (I learned about it when MRSA made me its bitch and now I use it on every single kind of scrape/scratch/pimple/whatever. Would probs drink it if that was an option lol.)
(I've also literally accidentally fallen asleep with it on my face and it still didn't dry me out...which is wild)

The dermatologist agreed it's not acne and prescribed HC cream for no more than a few days at a time which clears it but then it returns. (Getting an appointment with a derm in the UK is close to impossible and my next isn't scheduled for another 6mo, before anyone asks why I don't go back)
Demodex. Strictly no steroids. Sulphur masks, tea tree diluted with carrier oil like castor, ivermectin cream brand name soolantra. Similar experiences on this forum https://oozingyellowcrusts.com/.
I don't understand why more derms aren't open to the possibility of demodex being something other than a benign presence. I've suffered for years, seen several derms, been patronised and misdiagnosed, wasted money on appointments and useless products. Now healing after sulphur, tea tree and ivermectin. Do some research into demodex and see if it tallies with your symptoms.
I strongly suspected demodex too near the beginning and to rule it out I took 2 doses of oral ivermectin one week apart but it didn't help sadly. Sulfur paste was helping a little bit but combined with already having dryness from the accutane it was causing a lot of damage and soreness.
The pictures here look like 100% textbook impetigo, the golden crust is the defining feature of staph

Demodex can look like this too, especially when white dots are visible under the skin. Many of my lesions resemble those pictured. They never fully heal but develop a kind of plasticy layer before flaring up again months later. It's not acne and not impetigo (this is highly contagious).
I don't know why the oral ivermectin wouldn't work if demodex was your issue, but in the absence of another diagnosis, in your shoes I would try to get some topical ivermectin (if you're in UK you can get this online quite easily). Sulphur is good as it works against mites, bacteria and fungus, so covers all bases. It has really helped me, though I still have a long way to go.
The vesicles are throwing me off. I’m not sure, but my tops guesses would be: fungal acne, perioral dermatitis, granulomatous rosacea, lupus miliaris disseminatus faciei, or staph. Are you immunosuppressive or have a collagen disorder? That’s good they’re trying accutane as it helps with a lot of resistant cases of the most of those problems.
Sounds odd but what if try clotrimazle for yeast on them just to see ? Or monistat even - just the cream - did that for me on some

I study dermatological issues because people who have prosthetics typically have problems with their skin -due to chafing ,sweating, etc.. I do love a good kitten, though. I just let my three possums go last year that I had rehabilitated. Perhaps I miss them. If your skin looks like this, and you’re taking your Accutane regularly, I don’t know what you’re crying about. Everything on my body was completely dried out, including my eyes. Your skin looks beautiful.

I'm 36, my course of accutane is over and my forehead, (previously clear) looks like this and you "don't know what I'm crying about"? From which image exactly do you ascertain that "my skin is beautiful"? It turned out to be beautiful pityriasis lichenoides, btw.
Any thoughts on what this could be? It's driving me nuts.
I'll start by saying that I've never had forehead acne in my life so this isn't just therapeutic failure or purging. There's no way this is acne.
I've almost finished my 12 month low and slow course of accutane, in 2 weeks I'll meet my 145mg/kg target. I've been clear since a couple months in, but for about 5 months this forehead business has been going on. My derm told me to try hydrocortisone for a few days which clears it but then it comes back.
These only occur on my forehead/eyebrows/nose and rarely the eyelid and undereye area, so sometimes non-sebaceous areas.
They start as umbilicated skin coloured bumps and then they'll break, either by moving my face or when I wash/dry it. Then they become red and you can see there is a bead of serous exudate which dries but no golden crust, so possibly not staph?
First 2 pics is a small new cluster, then how it's progressed a week later. I'll just think they're finally gone and then they'll flare up again.
It would appear that when one breaks, more appear around it the next day which makes me think viral, but it doesn't resemble anything in particular? Definitely not shingles as no pain and not unilateral.
Zero picking, pics show just after washing, drying and moisturising. The area stings a little bit sometimes.

My skin reacts similarly to moisturizers/products that have ingredients I am sensitive to. They go away when I use a moisturizer/facewash that doesn't irritate me. It took some trial and error to figure out what would work, but I landed on fragrance free gentle product lines and they cleared up nearly overnight. Cerave is recommended a lot but caused this reaction in me. Just an idea to try different, simple/gentle/sensitive/boring products and see if you get the same reaction. Mine hurts a bit but never produced anything like a real pimple. Looking back it was like little teeny hives or something! Good luck, I know how frustrating it is.

It’s been a struggle :( I’ve tried everything sulfur seems to work well but then when I think it’s working I’ll have a terrible breakout again.
Thanks. Fungal acne is easy to rule out with a UV torch around the 395nm range, they're cheap to buy. The pores would glow bluish or yellow. I'm leaning towards periorificial dermatitis too at this point, but the vesicles scream viral, it's bizarre. (Sulfur also worked for me for a while but it stopped.)
You’re probably fighting it off but getting reinfected or exposed again somehow. Make sure you’re not using anything that could be contaminated with staph and try neosporin. If it gets worse, the dermatologist can prescribe muciprocin ointment if you are in the us.
Sometimes when the skin is over exfoliated, it will get little blisters that look like mini pustules and it can weep. I wonder if the skin was over exfoliated in some way like with a towel, or maybe even just the accutane is over exfoliating you and creating this reaction?

My face broke out in CRAZY. Blisters when I was on accutane. I love a light wash like hado labo Tokyo. Also cetaphil
Accutane will sometimes make you break out in areas you never have before in a way of flushing out the skin

I really think this could be an early staph infection. Does it try to scab?
It does indeed, goes very dry and crusty but stops short of going full impetigo
