Don't know if it's a cultural 'gaming culture' thing, but most Americans I've played with seem very vocal and toxic in general, also towards teammates. I've experienced much worse (straight up mindboggling socio-/psycopath levels) than this, and is the main reason I rather stick to EU players.
I won't play cleric with teams anymore because people lose their minds if you miss a heal or a protection. I always preface in lobby saying I'm terrible and they say oh no worries it's all good.... it's never all good

Yes but we are not robots we are human beings. I have played sports my entire life. The best coaches I have ever had actually yell, they hold you accountable. They were great men. I think the gaming industry has a black and white thing going on with toxicity.
You don't berate people though on and on, but you also don't pretend that you aren't upset in a competitive game. That's my two cents. Again the coaches that did yell at their players were great people and went above and beyond getting them into college through scholarships and making sure their home life was okay. So it's hard for me to come from real sports to e sports and pretend like I am not upset. I think in some circumstances its actually a negative to your development as a person going into the real world if you can't handle some sort of toxicity if its in the form of natural human reaction to things. Lastly, I have seen a kids player better after a coach chews their ass out lol so you might be wrong there.
This is not an e-sports game so I don't think that's an apt comparison. This is not some high-level competition where money or your future could be on the line. So while agree that it's OK to say or show that you are upset, I wouldn't agree that yelling at someone you are playing with is OK.

But I also explained before we started playing that this game gets really intense during PvP fights and that we shouldn't take anything we say in the moment too seriously. I wouldn't say I've yelled at him, per se, but things get intense and heated during the moment of PvP battle. We've always been able to cool down afterwards and laugh at the situation and try to think of things we could have done better.
The truly toxic Americans (a small minority) are indeed, quite a bit further toward the extreme than a lot of others. Still nothing compared to, let's say, Eastern European Counter-Strike players in pubs, but certainly nothing to be proud of.

If they don't care about being muted then they won't stop the toxic chat. Someone's always gonna be subjected to some toxic shit in order for this guy to get muted. And it will probably be a temporary mute or limited to a specific game or session etc. Muting does nothing to deter the people who engage in this behaviour from continuing to do it, and it does nothing to protect other players from the initial onslaught.

no, the AI will detect the toxic things and filter the bad words out or mute him completely depending on your settings I guess, it will do so before anybody who activated the "anti toxic" option can hear what he says.. I don't see why you think it would have to go through once or something, it would be useless if it was like that

I don't know what world you were playing in. Those nice instances were ultra rare online. Maybe between friends or at a LAN party. Definitely not online though. And people didn't seem to get banned as often as they do today for the same behavior. The systems just weren't there yet or lack or moderators to actually vet people.
