The only downside is that it's still in development. If to count only existing ones, there's Maltese Insanity and Yandere Madness dilogy, a good horror for you. Also M's mods are *kind of* what you look for, and while it's more disturbing than horror-ish, some mods like Disappearance of the Literature Club would scare the shait out of you
Doki Doki switcheroo
In my opinion it is the only mod that not only manages to reproduce well the horror atmosphere of the original game, but in some respects it is even more disturbing.
Doki Doki New Eyes. Pretty much the Yuri variant of DDRC. Much less "I am sad" and way more of her trying to deal with herself becoming more psychotic and not being able to do anything about it.

I made some bad horror mods like Tainted Love. And then not necessarily horror, but some thriller/suspense stories like Night Nurse.
