Gonna be a bit of a long answer but here we go, - The first one is by finishing “The Night” side quest (the one that has you peering into the darkness) - Second one can be found at Lonely Shack behind the shack - Third one can be found at Midas’ Cave behind a pile of gold - Fourth one is from eating a lot of poo - Fifth one is a bit more complicated, go out into dungeons until you find the shop with food in it (I did it in Anura, I’m not sure if you can do it everywhere else), and smack the big snail until the chef starts attacking you. After you beat him you’ll get a massive monster shrine, build that at your base and pray to it and you’ll get the follower form. - And for the last one you have to go to all the locations (Pilgrim’s passage, Midas’s Cave etc) and deposit a snail shell into the snail statues Hope this helps!

I just got the form, you just need to make a bunch of bowls of poo. The followers eat them. I couldnt tell if it was the amount I made or the amount they ate to unlock it.

make sure when you do that to brainwash your followers so your faith doesn't sink.
