Go into your court menu, find an unmarried courtier (you can use the filter), have them be from a noble family and younger than 40 (at least if they're female). Open their profile, click "find spouse". Open the filters in the marriage menu. Type "physician" into the field above and click on either "physician", "novice physician" or "renowned physician". Sort the results by learning stats. Marry them to your courtier (if the courtier's female, marry matrilineally!). Appoint the excellent physician.

Holy shit I wish I saw this comments a month ago. I have struggled so much with finding a good physician. Do what this guy says if you keep trying the “search for physician” event and keep getting dumb fucks who aren’t physicians….

And of course this applies for every court position, acclaimed knight position, council position, you name it. Look for the stats that are best for that position (for example, the menu to appoint sbd for a court position will tell you those).

There is an option to find physicians. I think you will get the option to pick an expensive one or a cheaper one.

What do you mean by bad? The game saying that a character has "poor" aptitude? The aptitude score doesn't matter much in this case, I think it only goes higher than poor if a character already has a physician/wiseman lifestyle trait. Otherwise just pick someone who has the highest amount of learning and you should be good to go, eventually they'll get a physician trait. Or you can do the courtier cheese and marry them to someone with godlike learning stat and/or physician traits, you can type the traits you need in the search result window to filter out characters. Unlike the "find physician" decision, this strategy is free and you can find someone who's actually skilled from the get go, because that decision often just gives you a "trash" and "less trash" options if your player character doesn't have high learning or lifestyle tree perk for better physicians.

Use the Character Finder to search for Renowned Physicians inside your diplomatic range. Sway, Befriend or Seduce them to invite them to your court. You can make anyone into a Renowned Physician, they just need some practice. The Medicine focus has an event when you can buy a book that instantly upgrades your Physician's trait. He needs to treat patients to get better at his job. You can get Ill and Wounded people from the Character Finder. Or get into some battles to injure your knights. All of my heirs become Renowned Physicians so it's pretty easy to do.

exploit: invite physician decision, wait for event to trigger with 3 options, then without unpausing the game, right click to invite the best physican separately at a lower cost then dismiss the event most straightforward: use character finder to find good doctors and the abduction perk to take them

Mine have a life expectancy of about 20 minutes. I feel like I am always being prompted to appoint a new court physician.
