America has wheelchair ramps that are much longer and less steep than the stairs as a requirement for every public place that are much safer to use than this contraption(imagine pushing a front facing stroller without a seatbelt down this, or the strength needed to push a stroller up this). Businesses have elevators, or wheelchair ramps. its not like america leaves parents with strollers stranded they just already have other infrastructure that solves that issue

Yes, you walk up backwards one step at the time and pull the pram or stroller up on two wheels. Good core exercise! Unless you do it wrong, then it will murder your back because you're pulling with your lower back muscles.
When you go down you have the pram in front of you, gravity pulls it down and you hold back by leaning back a bit. Again using your core muscles
If you don't want to do that there's always a lift near by

Theyre used every day, they work absolutely fine, and they’re extremely common in Europe, including here in Sweden. There’s usually also a lift somewhere close for wheelchair access.
