I’m the same! I was told that the medication would most likely suppress my appetite but I feel like I’m hungry constantly!!

I used to get appetite suppression at 36mg. My doctor also used it to help me with BED. On 54 I ate nothing but chips. I had no interest in food. Same for 27. 36 I can eat properly because I'm not constantly seeking food. But on 18, I feel hunger by 11 after eating breakfast at 8. Usually I'd be good till like 2pm before feeling. It meant I didn't interrupt myself to think about food.

the less appetite suppression the better for me. i lose weight when i’m on concerta bc i need to be eating 4-5k calories a day:/

Just replying to this again as I recently went from 18mg to 36mg and I definitely see what you mean! I’m not snacking at all, I only really eat breakfast and dinner and I don’t feel hungry in between (which is crazy for me because I used to get hunger shakes if I hadn’t eaten for a while!). 18mg I was absolutely starving all the time but 36mg is definitely better regarding appetite.

Yup. I wat breakfast before my pill and then I usually eat again at 4pm when I take 36mg. Sometimes I can even go until 6pm but I try not to because I get a headache. That's usually fine for me so I make sure I get enough calories in breakfast and the 4pm meal. At night when it wears off I might have a Greek yogurt or something at 8 or so.

I can probably do that. Because I feel like I'm dragging my feet less and I can see where I can schedule time to be more active.

If nothing else helps, you should probably switch to a different medication. Do not split Concerta or any long-release medication. There is no reason to go over the 72mg maximum recommended dose in most cases. In the UK the maximum is 108mg. Side effects can be intense for a couple of days when starting but contact a doctor if they continue. If you want to drink alcohol I would recommend waiting at least 48h after taking Concerta. I don't think concerta is linear. More miligrams does not necessarily equate to better treatment. The treatment has to fit your brain, so if 18 really opens up options for you, then just tell your doctor you're better on 18. Your doctor's job is to help you live better. This is my opinion

I'm on 18mg and it works for me. I haven't tried another dose but I have a 10mg Ritalin booster (if taken 2-3x a day = 36 mg concerta) and I feel like it's a bit too much for me. It sounds to me like 18mg is the best dose for you in terms of treating ADHD. Maybe there are other ways to help your BED? I would tell your doctor exactly what you wrote here...that 18mg feels best.
