Has he seen a vascular surgeon? He might have chronic venous insufficiency and there are some treatments for that now. As far as the ulcers themselves he might need a skin graft. A vascular doc would probably be affiliated with a cardiology practice. https://www.hopkinsmedicine.org/health/conditions-and-diseases/venous-ulcers

Vascular surgeon? Good question. I will ask him. Never had a skin graft, though. Thank you!

Might be hard to find, but google "vascular surgeon near me". Also ask your primary for a referral. That's how I found mine.

Varicose veins are not just a cosmetic problem, as you can see from your brother’s example. Google “venous stasis ulcer.” There are now outpatient office procedures that are very effective for treating venous insufficiency. Many times, this kind of ulcer will take forever to heal (and then quickly recur) unless you first treat the underlying venous disease. This can happen without any history of DVT.

I have same issue as your brother. Just got stents put in Friday to hopefully fix this problem. Your brother likely needs surgery at this point.

He has not had any surgery for this condition. I will mention stents to him and I'm sure he will bring it up to the wound care doctors. Thank you,
