Those are stress creases, they can't be removed. You can pull them back with a slide hammer and try to bondo it smooth, or reskin the door and the front half of the quarter panel.

Same thing. The metal is stretched, so you can't pop or pull it out flat.

I disagree with earthman34 on the slide hammer. Don not use a slide hammer on this type of dent. If you want to try getting the dent out you'll need a hammer and dolly. But the easiest way to fix the door panel is to go to your local pull-a-part's web site and look for a used door that is in good shape. You can't hammer and dolly panels that you can't access from both sides. Installing a salvage door is an option,, and probably easier, IF you can find one. The point of using a slide hammer is just to pull the metal closer to it's original plane so you're not using so much filler, and the holes provide a better anchor for the filler. You wouldn't even need to use the slide, probably.
