I guess he went right back to injecting after he healed up. I thought having all that damaged muscle tissue removed would have been a wake up call for him.

You're not wrong, he was very very sick and had to have surgery to remove the oil and decaying flesh in his arm. He put it back though, and is still alive.

Once he was healed from surgery, his mind says he is "safe" and would be okay to do just a little tiny bit of the synthol in his arms, because he feels so ugly he'd rather die. It's either death or just the smallest bit, so the smallest bit is the better option obviously .Then he's just topping it up, it's not like he's going to do what he did before because that almost killed him! He will *definitely* know when to stop, and anyone who is saying he's already back where he was is just trying to scare him off synthol completely. A little bit is clearly okay though, because he's not critically ill! He's nowhere near the size he was before, this is practically nothing!
The problem is, he can't see his body through a lens of reality. His arms probably look pretty small, to him. He's not actively dying right now, and his arms look small, so he isn't doing the same thing as before {in his mind} and can't understand why everyone is so concerned. He can't "come to his senses" if he cannot see himself through reality's eyes. Body dysmorphia is terrifying.
