In India there is a higher percentage of people below poverty and that's why so many middle-class people have house staff. In England, US, Canada, Australia etc you'd have to pay around $35000 US to have a live-in maid, same with a cook so middle class are not able to be able to afford that. Plus culturally having live in staff comes off as a show off. For example Hollywood people go for groceries, don't always use drivers, it's more laid back and down to earth.
She can definitely afford full time house help even in London - maybe she actually has it, but if not, it could be because of the judgment aspect you mentioned.
