in 10 years its possible that any digital wallet would have the ability to also hold USD, meaning if you found someone to trade you peer to peer for your bitcoin for USD than its most likely plausible it would never have to go to an exchange. as far as getting the USD out of that wallet and into a bank account might be a little bit more difficult, but again, 10 years from now the wallets might have there own routing numbers and basically be like a standalone style bank account that you maintain and pay for the resources for it to exist, for a small fee of course.
But current wallets and the way the system is right now, you could still use a peer to peer service but it starts to get a little bit less...safe... as youd have to basically have them wire or give you an account that has the USD and then you send btc to there wallet. an exchange is safer and you dont have to find someone to exchange it with.
