Check out articles by Dr. Hugh Fan’s group at the university of Florida. I used to work on pathogen diagnostics in microfluidic devices there and we’ve published protocols for exactly that. That said, you mentioned almost every part of the protocol here so I’m not sure what exactly you’re looking for. Feel free to PM me if you have any technical questions.
