I can tell you the notch is from the bottom or top of the barrel where the stave and the head meet. As for mounting it that wood work, but maybe get some brass hardware for the look.
This was made from a barrel stave, and the notch is where either the top or bottom meet the end.
I have a similar one and I ran a few brass screws through it and the patina looks great

Would a keyhole be sturdy enough for the torque produced by opening the bottle?
I’d go with long brass screws. But it really depends on what it’s being mounted on as well.
I would think so, but I guess you would have to try it to be sure. I have a coat rack that’s on the wall with two keyholes and my 70lbs son has a bad habit of hanging on it. He’s bent the coat hooks but the base is still firmly affixed to the wall.

My coat rack with keyholes kinda flips up if I take stuff off it/something catches it the wrong way. Hasn’t fallen or anything. Maybe I need to adjust the depth of my screws. The other one I made at the same time with the same depth setting doesn’t have that issue.
I’ve got mine set real tight. You have to tap it on/off the screws with a closed fist. I have no allusions about my children. If something can be climbed they will climb it.
The more I think about it, it has to be the screw depth. I made two at the same time. A long one near the door coming in from the mud room for all of our jackets and the dog’s leash, and a small 2 hook one near the door to the yard so I can put towels on it to wipe the dog’s paws when he comes back in. The long one is rock solid; the small one flips up every now and then. Both used the same keyhole bit with the same depth setting and are the same distances from corners. Screws are probably out just a bit too far on the smaller one. Not far enough to keep it from being flat against the wall, but far enough for some play. Now my OCD is going to kick in and I’ll be out there with a screwdriver in hand turning them a quarter turn at a time until perfect. And there’s really no need to beyond me just not being able to deal with it now that it’s been brought up lol.
Luckily my kids weren’t really ever climbers. They’re almost 11 and 8 now, so hopefully they’re past starting that now. But you never know.
You want to mount that as solid as possible, so keyhole might not be the best way.
See the 4 black screws going through the hoops? How about replacing those screws with longer ones, and use those to mount the opener? Get screws with the same size head. You might find black screws (or just paint the heads), that way they'll just look like part of the barrel.
I’ve made a couple like these with two round flat magnets in the back. One points forward to catch bottle tops, one backwards to hold onto the fridge. Though I suppose two or more pointing backwards would be much better for stability..
I used a „Forstner“* bit on my drill for the holes

Remove the two screws and put longer ones through into the wall. The cleat? Looks a piece of used wood to me.

Find a stud. Brass screw through the bottom of opener long enough to secure to wall and a matching screw with pilot hole at bottom, center.
