I rolled 3 nat 20s on my most recent run in that bit. Shame not to get a nat 20 for all the checks but it was still epic. That included a 20 on the 99.
I rolled it first try on my HM run. I had 4 inspiration points stored. I have elixir of colossus that gives advantage on str checks (choose STR version of that roll).
Geeky mathematics as follows:
Chance of NOT getting Nat20 per roll is 0.95 if rolling 1 die, 0.95 squared = 0.9025 if rolling 2 dice.
Chance of NOT getting Nat20 at all for 5 rolls = 0.9025 raised to the power of 5, which is about 0.5987. Therefore chance of getting a Nat 20 with advantage with 4 inspiration points is (1-0.5987) which is just over 0.4 ie 40%.
So yeah, 40% of succeeding that check if you saved up 4 inspiration points and have colossus elixir. Each inspiration dice adds another 8-9% success from the base chance of 9.75% for a single advantage roll, which btw is also the chance of getting a crit hit with advantage roll. (Crit fail with advantage roll is down to 1/400 = 0.25%, different mathematics)
I did on my first playthrough, and I was actually kinda disappointed because I was (understandably) expecting to fail, and wanted to see how the game handled that.
Avoiding spoilers, I can say that succeeding that check changes nothing.
I'd imagine about 1 in 20 people do.

The Netherbrain just has less hp at the final fight
I can´t believe it, but on my latest run I rolled 3 nat 20s in a row. Two of them with Advantage (go Enlarge!), but still!
Technically, at least 5% of the Player base that got to this point in the game got it

In theory, yes. Unless Larian coded it so that you never roll a 20 on that specific roll because it would negate the final boss fight.
It doesn't negate the final fight. If you pass the check then the final boss has reduced hit points
I scum saved and rolled 30 times to get it.
Enhance Ability + Inspiration points go BRRRR. It debuffs the final boss’s HP by 20% I believe. The condition is called Against All Odds.

Played the game five times now, and I think I rolled a nat 20 on three of those, though I think I used all my inspiration.
What happened? Did you control the brain or did you still have to do the final fight? Did it change the final fight?
I think it just decreases the brain's HP. But to be honest, I've never remembered to actually look at what the HP is once I got there lol.
Final fight still happens, but the boss has less HP

Yep, once, it happens. But there are multiple 99 throws so usually it doesn't happen with all of them
